Nutritional Assessment

Note: This Nutritional assessment will provide you with a complete review of your current nutritional health and recommendations on how to improve your nutritional health.

To get started, please fill out and submit the online Consultation Request Form. A Carie Boyd’s Prescription Shop pharmacist, who specializes in teaching people the importance of adequate nutrition and vitamin supplementation, will review your consultation request and will send you an email with a link to a questionnaire. The pharmacist at Carie Boyd’s Prescription Shop will contact you upon receipt of the completed questionnaire to finalize the execution of the nutritional assessment.

The fee for this nutritional assessment is $100.00 USD.
